Here’s What You’ll Do for Our Residents & Facility:
Conducts Psychosocial Assessments: Evaluates a new resident's emotional, social, and psychological history.
Develops Care Plans: Collaborates with the healthcare team to create and maintain individualized care plans & updates the interdisciplinary team on resident progress and goals
Emotional Support: Provides counseling to help residents cope with grief, loss, depression, anxiety, and the transition to a nursing home.
Advocacy: Protects and promotes resident rights, ensuring voices are heard and needs are met.
Grievance Resolution: Mediates complaints and reports any suspected abuse or neglect.
Discharge Planning: Coordinates safe discharge plans, connecting residents with community resources like home health care or hospice.
Resource Connection: Assists with connecting residents and families to legal, financial, and insurance case management.
End-of-Life Support: Guides residents and families through advanced directives
Does This Sound Like You?
Licensed to work within the State of Connecticut as a LSW Licensed Social Worker
Able to pass background check, drug screen, professional references check & physical evaluation
High level understanding of leadership, customer service and clinical competency
Leadership experience in a long-term care setting
